Hi
From where I stand and whom I talked to lately, I don't think a lot of PR agencies are doing what you described when it comes to bloggers....I see a lot of 'rough targeting' and 'same email for everybody'. Granted there are quite a lot of niche bloggers out there compared to journalists that are well know so it's a bit more complex to implement the process you described. Nevertheless, it should be the rule and not the exception. I preach what you described, hopefully more people will follow and 'spam pr' will be gone!

One more thing I don't see on your list: Participate in relevant communities. i.e: a corporate blog should be fairly focused and outside driven, there' re typically 100s of similar blogs out there. Identifying them, listening/engaging/developing relationship with those is an important part of any social media activity.

Good list of mistakes.
#3 speaks particularly to me because I think it's the core issue I see around. I think to do #1 well, you need #3. To avoid #4, you do #3 and so on....
Listening to millions isn't listening, it's 'monitoring' without a true intent to engage. Indeed one can't listen effectively to more than 150 individuals over time, and certainly, just a few at a given time like in the real world (i.e: listen with a desire to know and engage with some kind of relationship building intent).
I fully advised a targeted listening attitude toward the 'communities and people' who matters to your brand (they may be a few 100s of those max to handle for a 3-4 marketers team). It's the best use of marketers scarce time and resource for optimum ROI.
